In-home care after a hospital stay or surgery

The first days home from the hospital are when people are most likely to fall, miss medications or skip follow-up appointments. Families are often handed a stack of instructions and expected to manage alone.

CareBridge caregivers help your loved one settle in safely, follow the discharge plan day to day, and tell you quickly when something does not look right.

Licensed Georgia private home care provider. Private pay. Serving 9 metro Atlanta counties.

How our caregivers help

  • Help getting home and getting settled on discharge day

  • Bathing, dressing and safe movement around the home

  • Meals that follow the discharge diet

  • Medication reminders that follow the discharge instructions

  • Encouragement with exercises the physical therapist assigned

  • Rides to follow-up appointments and the pharmacy

  • Watching for changes and telling the family and the care team quickly

  • Overnight or short-term schedules while recovery is most fragile

A good fit when

  • Coming home after a hip or knee replacement or other surgery

  • Recovering from a stroke, a heart event, pneumonia or a fall

  • A family member who cannot take time off work for the first weeks

  • Anyone whose discharge planner says they should not be alone at first

Good to know

This is non-medical home care. We do not provide nursing, wound care or therapy. If the doctor has ordered home health, our caregivers work alongside that agency and fill the hours between their visits.

What the first week looks like

  1. 1

Call us before discharge if you can, so care can be ready when your loved one comes home

  2. 2

We read the discharge instructions with you and build them into the care plan

  3. 3

A caregiver is introduced and scheduled around the hardest times of day

  4. 4

We review the plan as recovery improves and step hours down when it is safe

Post-Hospital Recovery Care: common questions

Can care start the day of discharge?+

When a caregiver is available in your area, yes. Call (404) 917-3101 as soon as a discharge date is known so we can complete the consultation and line up a caregiver.

Is this the same as home health?+

No. Home health is skilled medical care ordered by a doctor, such as nursing or therapy visits. CareBridge provides non-medical home care: the daily help between those visits.

How long do families usually need recovery care?+

It depends on the recovery. Some families need a few weeks of daily help and then step down to a few visits a week. We adjust the plan as your loved one gets stronger.

How do families pay for care?+

CareBridge is a private pay home care agency. Families pay directly, and we also work with long-term care insurance and veterans benefits where they apply. We do not accept Medicare or Medicaid, including the NOW and COMP waivers.
